A total of 14 individual spaces for children's personal items were located in the center, which is not adequate for 57 children enrolled in care at the center.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One (1) bottle of 91% rubbing alcohol, one (1) gallon jug of Covalent Clean hand sanitizer refill, one (1) gallon container of Neutral disinfectant, one (1) gallon jug of Clorox and two (2) bottles of Steramine tablets-with keep out of reach of children and additional warnings on the labels, were found the unlocked kitchen, accessible to children. There was one (1) bottle of liquid white out on a shelf, beside the desk in the lobby area.

The group leader employed on August 15, 2019, had a medical statement on file dated October 31, 2019. One bus driver employed on November 28, 2018, did not have a medical statement on file.
