A current activity plan was not posted for each group of children for reference. The activity plan posted was dated 3/20/23-3/24/23. The staff member stated that she dated it incorrectly. The Assistant Director corrected this during the visit.
2023-03-27 Violation 539 .2508(e)(1-5)
When screen time was provided to school-aged children, it was not offered as a free-choice activity; not used to meet a developmental goal; was not limited to 30 minutes per day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Screen time log to document screen time usage was unable to be located. The Administrator instructed the assistant teacher to write it down on a piece of paper.

Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Students did not wash hands upon arrival into the classroom from outdoors.

Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. Staff did not wash hands upon arrival into the classroom from outdoors.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Five (5) outlets were uncovered in space #1A and seven (7) outlets were uncovered in space #1B. The assistant teacher and assistant director corrected this during the visit.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Zep Clean EMS Spirits Disinfectant Wipes were located below five (5) feet. The assistant teacher moved the wipes above five (5) feet during the visit.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Debris, trash, and broken equipment were located on the outdoor play space. Broken plastic storage containers, broken plastic helmet, plastic bag, and trash located on the outdoor space. The Assistant Director picked up some of the debris.

The location of the first aid kit was not shared with staff. When asked to see the First Aid kit, the staff was unable to locate it and stated that they did not know where it was.

At least one staff member was not present who had successfully completed First Aid and/or CPR training when children were in care. When I arrived at 2:50p at least one (1) staff member who had successfully completed First Aid and CPR was not on-site.

The person identified as the person or alternate person responsible for carrying out the emergency medical care plan was not on the premises at all times and/or did not accompany children for off premise activities. When I arrived, the staff member responsible for carrying out the emergency medical plan was not on-site.

All equipment and furnishings were not in good repair. There was chipping paint and rust on the black metal fence.
2022-10-19 Violation 807 10A NCAC 09 .0601(a)
A safe indoor and outdoor environment was not provided for the children. The fence in the outdoor space has several areas with holes and fence boards that are bowing out from the fence post. There were also holes in the fence that were sharp that a child could stick their arm in and get stuck or injured. One (1) hole a child could potentially try to crawl out and get stuck or on the outside of the fence.

Gates to the fenced outdoor play area did not remain closed while children occupied the area. The gate to the left as you enter the classroom was unlocked and opened to where the AC unit was located in that space.
