In the bathroom used by children there were one 13.5 oz. bottle of oatmeal soothing shampoo for dogs, and one 3fl oz. bottle of Dr. Teal's Foam Bath. both labels read keep out of reach of children. The bottles were removed during the visit.

The additional caregiver(J.B.)start date was 6/1/22 did not have TB prior to starting. The TB was received on 7/18/2022.

Operator did not complete the required number of on-going training hours as specified in rule. The operator had to complete eight hours of on-going training and has only completed 4 for 2022. The operator still has to complete 4 more hours.
