Description: Cheryl Phillips is a Registered Family Child Care Home in Baltimore MD, with a maximum capacity of 8 children. The home-based daycare service helps with children in the age range of 0-23 Months, 2 years, 3 years, 4 years, 5 years, 5 years to 12 years. The provider does not participate in a subsidized child care program.

|Findings: Upon inspection, provider toilet needs repair. A part to repair it had been received during the inspection; provider should send statement of correction once part is replaced. Send correction to the OCC ASAP.|
|Findings: Provider has areas of the home that have peeling paint or holes in the wall. The bathroom has peeling paint and the hallway outside the bathroom has a hole in the wall. Provider will ensure that they are repaired and repainted within one month and will let specialist know when she has done so. Provider's screen door to the child care area currently has no door knob, so substitute had to reach through a hole in the screen to open door. Provider will ensure that door knob is replaced and screen is repaired within five days.|
|Findings: When specialist arrived, there was a substitute caring for the children. She appears to be very familiar with the provider's set up. There is no substitute log to observe. Substitute was not sure how many days she has subbed for the provider in the last year.|
|Findings: Provider has two children in care who do not have part one of the health inventory filled out and included in the files. Provider will have parents fill out form and let OCC know that she has done so within 5 days.|
|Findings: Specialist observed only 9 hours of continued training in provider's folder. Provider was not available at inspection to provide any other certificates, but she will give specialist any other certificates needed to make up the extra 3 hours of continued training required. Please send to OCC within 5 days or send a plan of correction.|
|Findings: Provider has three children in care who do not have a lead screening included in file. Provider will obtain lead screening results for those three children and include them within their file within 2 weeks. Provider will let OCC know when she has done so.|
|Findings: Provider is missing paper towels, tape, and fragrance free soap from her first aid kit. Provider's flashlight does not currently work. Provider will ensure that she has all items needed for her first aid kit and all are in working order within two weeks and will let specialist know when she has done so.|
|Findings: Provider does not have any documentation that she has given the Parent's Guide to Regulated Child Care to parents. Provider will add to her contract/handbook or find a way to have parents sign off on it. Provider will submit to OCC documentation that each parent was given the Parent's Guide to Regulated Child Care within 5 days.|
|Findings: Provider has several cleaning items, including bleach and dishwasher detergent, on the floor of the kitchen. The kitchen is included on provider's license as an approved child care area. If provider wishes to restrict the areas of child care, specialist will change her license to reflect the rooms that she is using for child care. Specialist recommends placing cleaning items in childproof cabinets or cabinets out of the reach of children.|
|Findings: Provider does not have a written record of daily attendance verified by parent. Provider's record is incomplete and undated. Provider will find a new way to keep attendance records and let OCC know that she has done so within 5 days.|
|Findings: Provider has recorded the fire drills, but does not have any emergency drills recorded. Provider will ensure that she is conducting and documenting emergency drills twice a year. Provider will submit documentation of at least one emergency drill and a plan of correction within the next two weeks.|
|Findings: Provider's first aid and CPR certification expired in February of 2019 and substitute or specialist could not find updated information. Provider will provide updated CPR information within 5 days.|
|Findings: Two children in care were missing information on their emergency cards. Provider will have parents fill out emergency forms completely and submit to OCC within 5 days.|
