Caregiver agreed that the dog will be kept in a back room not used for child care and will not be accessible to children. When the dog need to be in the outdoor area, children will remain indoors. Caregiver will supervise the dog closely during transitions between the back room and outdoor area.

Dog, Priscilla, was observed in the front room with children today and roaming freely. The dog scratched a child during the monitoring visit of 11/28/18 and showed aggression to licensing (growling) on both 11/28/18 and today. Caregiver is not following the plan of correction established on 11/28/18 to keep the dog inaccessible to children in care.

Child toilette in a potty chair in the front play room (living room) after which caregiver cleaned the child and changed child's diaper on a non-porous pad on the floor. The pad used is one that allows liquid to soak through; the type used for beds in hospitals.

Three dead roaches were also observed in plastic bottles sitting on the kitchen floor (the bottles contained a small amount of red liquid described as 'sugar water').
