Noncompliance Area: Staff person #3 did not receive training regarding the emergency plan on an annual basis during the period between 1/26/2025 and 3/16/2026.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: A protective receptacle cover was not placed in an electrical outlet on the blue wall accessible to children 5 years of age or younger in the game room.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: The following toxic materials labeled keep out of the reach of children were observed in areas accessible to children in the game room: a bottle of hand sanitizer and packet of sunscreen were observed on the storage cube and (2) bottles of hand moisturizer were observed on top of the desk.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Peeling paint/damaged plaster was observed on the handrail on the steps to the entrance of the facility, on the walls in the makers room and on the ceiling in the multi-purpose room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: A visual strip or other visual identification was not placed on the glass doors used to enter the building, the gym, the pre-K room and the makers room, and the glass window between the gym and the game room.

Description: Handwashing signs

Noncompliance Area: A handwashing sign was not posted at the sink and at the toilet in the bathroom in the office.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
