Findings: The Team Leader observed that the diaper changing surface in the 1's classroom had several holes where safety straps had been previously anchored to the diapering surface. The holes created a surface that could not be completely sanitized.

Findings: The Team Leader observed the following on the outdoor activity area. Wire screening around the garden had a section of wire that had sharp ends. Areas of the wood barrier around the garden had spintery wood that was peeling away from the other wood. Several metal anchors on outdoor equipment were exposed.
