Findings: The Team Leader observed that an outdoor activity area had been developed and was being used by enrolled children. The licensee failed to submit a written request for approval at least 30 calendar days before the intended date of using the new outdoor activity area.

Findings: The Team Leader observed that several children in the 2's room were in diapers. The 2's room does not have an approved diaper changing area. Staff in the 2's room said about half of the children in the room are in diapers or pull-ups and are brought to the 1's room diaper changing area to be changed.

Findings: The Surveyor observed that the fall zones under the climbers on the upper outdoor activity area had less than 6 inches of resilient material.

Findings: The Team Leader reviewed the facility children's attendance forms. Child # 1 lacked admission times and admission signatures for 2/3, 2/5, 2/6, 2/11. and 2/12/2020. Child # 2 lacked release times and release signatures for several dates in February 2020. Child # 3 had admission and release signatures using a first name only.
