2022-11-22 Mandatory Review 13A.16.06.12B(1) Corrected
Findings: One Aide only completed 5 of the 6 required continued training hours for the annual training cycle. Aide has completed an additional 6 training hours for the current training cycle. LS used 1 of these hours to correct the shortage of hours from the last cycle. This has been corrected.
2021-11-12 Full 13A.16.03.04E Corrected
Findings: Center has 39 children enrolled. All emergency forms were reviewed. 15 children's records were reviewed. Of the 15 records reviewed, the following children meet the birthdate requirement for blood lead testing and do not have results in their record: CBren, MP. Please obtain from parent/guardian and add to children's files. Send copies to LS.
2021-11-12 Full 13A.16.03.04G Corrected

2021-11-12 Full 13A.16.03.05B Corrected
Findings: Licensing Specialist observed posted staffing patterns to not be current upon arrival to the center. Center is reminded staffing patterns are to be current, complete and posted. Director had current staffing patterns in the office and posted them in the center during inspection. This is corrected.
2021-11-12 Full 13A.16.03.06A(2) Corrected

2021-11-12 Full 13A.16.10.01A(3)(c) Corrected
Findings: The center did not have a fire drill logged in December 2020 and it appears the center has only been doing one disaster drill per year for the last few years. Center is reminded fire drills are to be practiced and recorded at least once per month and disaster drills should be practiced and recorded at least twice per year. Please send written plan of correction. *LS received written plan of correction the same day as inspection. Fire drills have been planned for November and December 2021 and a disaster drill has been planned for December 2021.
2021-11-12 Full 13A.16.11.04F(1) Corrected
Findings: Upon LS arrival at the center, Director was not present, nor was the other employee who has medication administration training. The center was without MA coverage for approximately 20 minutes and then Director returned and this was corrected. Staffing patterns show coverage from 7am-5pm (center's current operating hours), but only 2 employees have completed the MA training, so if they are both absent from the center, there is no coverage. LS strongly recommends having at least one, preferably two, other employees complete the MA training.
2020-11-30 Mandatory Review 13A.16.03.06A(2) Corrected
Findings: One employee left employment and OCC was not notified in writing with 5 business days of the employee leaving. Center is reminded to provide written notification to the OCC when employees leave employment. Corrected.
2020-11-30 Mandatory Review 13A.16.07.06C Corrected
Findings: LS observed a new employee, KW, alone in charge of a group during inspection today. OCC has not received federal or State criminal background checks or a release form to conduct a review of child abuse and neglect records. This is the employee's first day at this center and was with the group during nap time while another staff member was on break. The other staff member did come back during inspection and went into the classroom. Center is reminded that no employee should be left alone with children unless all criminal background checks have come back and the release form has been cleared by OCC. LS did receive a copy of the release form for this employee by email later in the day and was able to review child abuse and neglect records. Please continue to 'shadow' this employee until full criminal background checks are received by OCC. *Corrected
2019-11-25 Full 13A.16.03.06A(2) Corrected
Findings: Two employees ended employment and OCC was not notified within 5 working days. *Corrected.
2018-11-19 Mandatory Review 13A.16.05.12E Corrected
Findings: Licensing Specialist observed one baby swing on the toddler playground to have a crack in the base. Swing was replaced during inspection..
