Noncompliance Area: At the time of inspection, the loose fill (rubber chips) used as a protective surface measured a depth below the recommendations of the USCPSC (6 in of loose fill). The loose fill depth was measured at .75 inches to 2.75 inches.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Description: Infant/toddler - no material less than 1 inch

Noncompliance Area: At the time of inspection, the outdoor playground area where infants and toddlers play included rubber chips that were less than an inch in diameter.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care.

Noncompliance Area: At the time of inspection, Child #1 and #2 did not include the address of the individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Description: Each child care space

Noncompliance Area: At the time of inspection, emergency contact information was not present in any child care space for any children receiving care in the space.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Description: Excursions

Noncompliance Area: At the time of inspection, the provider did not have all emergency contact information specific to each child available and accompanying staff and children on each excursion in the "To Go Bag".

Correction Required: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Noncompliance Area: At the time of inspection, the files for Child #2 did not include a child health assessment.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Description: Emergency plan

Noncompliance Area: At the time of inspection, emergency plan did not include accommodations for evacuation of children to a location away from the facility premises.

Correction Required: The facility shall have an emergency plan that provides for accommodations for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by §3270.94 (relating to fire drills).

Description: Fire safety - 1 yr.

Noncompliance Area: At the time of inspection, Staff members #1, #2, and #3 did not have verification of participation in annual fire safety training.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Description: Health and Safety Training - Pediatric First Aid and CPR

Noncompliance Area: At the time of inspection, Staff member #4 did not have a Pediatric First Aid /CPR training from approved provider.

Correction Required: Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.

Noncompliance Area: At the time of inspection, there was a container of hand sanitizer within reach of children on the changing table in the Infant Room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: At the time of inspection, water in the children's bathroom sink was measured at 120 degrees F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F.

Noncompliance Area: At the time of inspection, the First Aid kit used for taking to the children to the playground did not include tweezers and scissors.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Description: Review policies and procedures/Given parents in writing

Noncompliance Area: Written parental information to be given to parents at the time of enrollment did not include the facility's policies on supervision.

Correction Required: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. At the time of enrollment, a parent shall receive in writing the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. Amend the parent handbook to include the specified written policies in accordance with 55 PA Code 3290 regulations. Submit a copy of the amended parent handbook/information for verification of compliance.
