Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. On the day of the inspection a child was observed running around with a pencil in his hands and then proceeded to stab the garbage can repeatedly. Staff did not notice or do anything to stop him.

Violation Observed: Ensure that adequate procedures for tracking children are adhered to at all times throughout the center. Several staff members were asked if they knew how many children were in their care at the time of the inspection and none of them were aware.

Violation Observed: Ensure that adequate staff/child ratios are maintained at all times throughout the center. Multiple times staff members walked out of the classroom without ensuring that ratios were maintained.

The requirements for co-location of a center within a multi-use building are as follows, during operating hours, the Office of Licensing may require the center to operate in a separate room, floor, or section of the building.

Violation Observed: Ensure that the center adheres to all requirements to prevent co-mingling in multi-use buildings, including operating in a separate section of the building. Ensure that other school programs do not utilize the same rooms as the licensed center has been approved for at the same time.

On the day of he inspection children were not given snack. Snack was not offered to children and those that had some home snack ate as they walked around the room.
