Corrective Action: Ensure direct supervision is maintained at all times. LS observed a 2nd-grade classroom in which a child left the room unaccompanied to use the bathroom. LS Informed staff in the room that they must accompany children to any part of the facility and maintain a staff-to-child ratio while doing so. LS observed staff at the front desk radioing to the classroom for a child, then a child left the classroom to meet their guardian at the front door, with no staff in the hallway with the child. LS observed a child leave a back classroom, run down the hallway to the front door, and give another child something that was left in the classroom. No staff were in the hallway with the child.

Three staff members need health appraisals with TB results. Three staff members need a release of employment history completed. Four staff members need attempts for service letters.

Corrective Action: Ensure staff administering medication have the Administration of Medication certification. Ensure that a Medication Administration Record (MAR) is completed, that MARs are correctly documented, that medication is labeled in its original package with the pharmacy label, and that medication is not expired. A staff member administered medication on 1/12/26 and 1/16/26. The certification to administer medication expired and was recertified on 2/17/26. A staff member administered medication on 12/22-23/2025, 12/29-30/25, and 2/26/26 and was not certified until 2/28/2026. 17 medications on site have no MAR. 10 MARs on site were not completed correctly. Five medications have no names of the medications. One child no longer attends, and the medication expired on October 14, 2025. Three medications have expired.
