Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In classroom 5 over the counter medication was not kept in locked storage. This was corrected when the medication was removed from the classroom and stored in locked storage.

All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. A hole in the wall was observed in the classroom for school agers. The hole was large enough that a child could put their hand in it.

Styrofoam was accessible to children under the age of 3 in space #6. This was corrected when the styrofoam was removed from the classroom so, it's not accessible to children.
