The infants were moved during the visit. Label all cots in the preschool room. A child's cot was not labeled or available to him on his second day of attendance.

Corrective Action: Cell phones use in the classroom is prohibited. A staff member was watching a video or TV on her phone when she was preparing an activity for the children while 8 of the 9 children were sleeping.

Repair or replace the carpet.

A staff member stated to a City of Wilmington official the fourth floor had been used as housing for other people. People are not to reside in the center. Corrective Action: Smoking is prohibited at all times. Evidence of smoking was detected during the investigation visit. Cease immediately.

Corrective Action: All outlets in the toddler area need to be covered. This was corrected during the visit. Adequate lighting is needed in the infant hallway, toddler hallway, and 2A classroom. Repair/replace lighting. Flooring must be cleanable at all times. Replace flooring in the one-year-old’s room and both bathrooms on floor 1.
