Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. The infant was awake and observed in a crib/bassinet for approximately 24 minutes before he was moved. When he was moved, he was moved to a battery-operated rocking crib. Toys and materials developmentally appropriate for the infant were not provided during the visit.

Children did not have the opportunity for outside play each day that weather conditions permit. The provider reported that children do not go outside every day.

Each day, a child aged 12 months or younger was not given the opportunity for supervised play while positioned on his/her stomach while awake and alert. The operator reported that the infant only has tummy time while the other children are resting/napping. There was not enough floor space in the childcare room for three (3) cots and a space for tummy time for the infant.

Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. The HVAC unit in the outdoor space was accessible to children.
