Noncompliance Area: Staff person #3's pediatric first aid and CPR training expired at the end of September 2025. Staff person #3 has completed Pediatric first aid and CPR training, however the trainer's PQAs approval has expired. The training was completed in January of 2026, but the trainer's PQAs expired in July of 2025.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Until such time as the required training has been completed, staff person #3 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. Noncompliance Area: Staff person #1 was observed changing a diaper. After the diaper change, the staff person did not wash her hands, and also did not wash the child's hands.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Noncompliance Area: Staff person #1 was observed changing a child's diaper. The diaper changing surface was not cleaned after the child's diaper was changed.

Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

(1) Prevention and control of infectious diseases (including immunization) and the establishment of a grace period that allows homeless children and children in foster care to receive services under this subchapter while their families (including foster families) are taking necessary action to comply with immunization and other health and safety requirements. (2) Prevention of Sudden Infant Death Syndrome and use of safe sleep practices. (3) Administration of medication, consistent with standards for parental consent. (4) Prevention of and response to emergencies due to food and allergic reactions. (5) Building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water and vehicular traffic. Noncompliance Area: Staff person #1 has been employed for more than 45 days and did not have a complete DHS FBI clearance on file. Staff person #1's state police clearance was for volunteer purposes only. (CORRECTED ON SITE) Staff person #2 has been employed for more than 45 days and did not have a complete state police clearance on file. (CORRECTED ON SITE) Staff person #3 began employment with a PDE FBI clearance on file, in lieu of the required DHS FBI clearance. The correct clearance was obtained 8 day after start date. Staff person #5 did not complete a mandated reporter training within 90 days of their date of hire. The training was completed 141 days after the date of hire. At the time the staff person #6 began work in the facility, her record did not contain the applicable completed out-of-state child abuse clearance information required under the CPSL and did not contain a copy of request for the out-of-state child abuse history clearance required for provisional hiring under the CPSL. The clearance has since been obtained. Staff person 7 - Clearances were not updated timely during the previous year. The state police clearance expired 9/9/24, but was not updated until 12/4/24. The child abuse clearance expired 9/27/24, but was not updated until 12/11/24. The DHS FBI clearance expired 9/13/24, but was not updated until 12/18/24.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person #1 may not work in a child care position at the facility until complete clearances are on file.

If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. Noncompliance Area: Mulch (less than 1 inch in diameter) was observed on the the fenced in playground that is used by toddlers.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care. Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Description: Age and Training/General requirements for facility persons

Noncompliance Area: At the time of inspection, none of the eight staff that work at the facility had a current pediatric first aid and CPR training. Staff persons 1 and 8 have the following training card on file - CPR and AED (did not include pediatric first aid). Staff persons 2, 3, 4, 5 and 6 have the following training on file: "CPR, AED, and Basic First Aid, adult, child and infant". The cards state, "This program is not designed to meet pediatric first aid regulatory requirements." Staff person 7 has the following training on file "CPR/AED/First-aid (Adult / Child / Infant / Choking / AED / Injury & Universal Precautions). This training does not meet the requirements of a "pediatric first aid and CPR" course.

Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). The PA state police clearances on file for staff person 4 were dated 5/2016 (expired 5/2021) and subsequently 1/6/2022. The update was not obtained timely (within 60 months / 5 years). The record for staff person 5 did not include a mandated reporter training that has been approved for child care purposes. A partial training was on file for PDE purposes. Staff person 5 has been employed for more than 90 days.

A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
