Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WAS AN UNSTABLE EASEL WITH LOOSE AND MISSING SCREWS IN THE LOWER CHILD CARE SPACE.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: DURING THE RENEWAL INSPECTION THE PROVIDER DID NOT HAVE A CERTIFICATE TO VERIFY THAT THE CRIB WAS IN COMPLIANCE WITH THE US CONSUMER PRODUCT SAFETY COMMISSION.

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement.

Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WAS A LOOSE PLASTIC TRASH BAG HANGING BESIDE THE TOILET IN THE BATHROOM IN THE LOWER CHILD CARE SPACE.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: DURING THE RENEWAL INSPECTION THE DIAPER CHANGING AREA DID NOT INCLUDE A PLASTIC-LINED, HANDS-FREE, COVERED CAN.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: DURING THE RENEWAL INSPECTION THE HEALTH ASSESSMENT FOR STAFF 2 DID NOT INCLUDE AN ASSESSMENT FOR COMMUNICABLE DISEASE OR SUITABILITY TO PROVIDE CHILD CARE.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: DURING THE RENEWAL INSPECTION MILK, CORN AND CHICKEN NOODLE SOUP WAS NOT REFRIGERATORED FOR OVER AN HOUR BEFORE BEING SERVED FOR LUNCH. AN OPEN JAR OF APPLE SAUCE WAS ALSO UNREFRIGERATED FOR OVER AN HOUR BEFORE BEING PUT AWAY.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WAS NO SMOKE DETECTOR ON THE 2ND FLOOR OF THE HOME. THIS VIOLATION MUST BE CORRECTED WITHIN 5 DAYS FROM DATE OF VIOLATION. CHILDREN MAY NOT BE IN CARE UNTIL CORRECTED.

Noncompliance Area: DURING THE RENEWAL INSPECTION THE FILE OF STAFF 1 AND STAFF 2 DID NOT INCLUDE VERIFICATION OF EXPERIENCE PRIOR TO SERVICE AT THE FACILITY.

Correction Required: A primary staff person shall have attained a high school diploma or a general educational development certificate and 2 years of experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: DURING THE RENEWAL INSPECTION CHILD CARE WAS BEING PROVIDED TO 9 CHILDREN IN THE KITCHEN.

Correction Required: A facility shall provide indoor child care space for individual and group small muscle activity. Measured indoor space excludes space occupied by halls, bathrooms, offices, kitchens and locker rooms.

Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WERE 5 CHILDREN IN THE FIRST FLOOR MIDDLE ROOM EXCEEDING THE ROOM CAPACITY OF 3.

Correction Required: The capacity established for an indoor space may not be exceeded.

Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WERE SEVERAL ELECTRICAL OUTLETS ACCESSIBLE TO CHILDREN THAT DID CONTAIN PROCTECTIVE RECEPTACLE COVERS.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: DURING THE RENEWAL INSPECTION THE WATER TEMPERATURE IN THE BATHROOM MEASURED 120 DEGREES.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WAS A LONG CRACK IN THE OUTSIDE WALL SEVERING THE WALL. IN THE LOWER CHILD CARE SPACE THERE WERE TWO ELECTRICAL OUTLETS THAT WERE LOOSE WITH GAPS BETWEEN THE WALL AND THE OUTLET. IN THE BATHROOM THE SINK WAS COVERED WITH PAINT AND THE FAUCET WAS CORRODED. THE PIPE COVER BEHIND THE TOILET WAS RUSTED AND LOOSE EXPOSING A HOLE AROUND THE PIPE. THE SINK IN THE FIRST FLOOR BATHROOM WAS BACKED UP AND NOT DRAINING PROPERLY. THE WOODEN CHANGING TABLE CONTAINED CHIPPED MARKS ALONG THE SURFACE AND THE DIAPER CHANGING PAD WAS TORN.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: DURING THE RENEWAL INSPECTION THERE WAS A TABLE AND SIX CHAIRS IN THE LOWER CHILD CARE SPACE WITH PEELING PAINT. ON THE FIRST FLOOR THERE WAS DAMAGED PAINT ON THE FRONT DOOR, AROUND THE DOOR FRAME AND ON THE FLOOR ACROSS THE THRESHOLD.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: During the inspection on 10/15/18 the file records for child #1 did not include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable
