Violation Observed: Ensure that all indoor and outdoor equipment is sturdy, safe, free of hazards and used in accordance with the manufacturer's instructions; wherein two children in Room 1 ages 15-18 months were observed seated in Power Swings; one was trying to get out. The manufacturers instructions state that the use of these should be discontinued when a child becomes active and can climb out.

Violation Observed: Ensure that the center complies with the conditions of the license including the age restrictions for specific classrooms, wherein children under 2.5 were observed in a lower lever classroom (room 3). Children 0-2.5 are limited to classrooms 1 and 2, on the first floor only. 6/20/23-Unable to abate violation as children under 2.5 remain in lower level classroom. 7/19/23- Unable to abate violation. Some children were relocation upstairs, however, at least two children under 2.5 remain in the lower level classroom. Center must submit a Plan Of Action to the OOL detailing steps center will take to ensure all children under 2.5 will be relocated and remain in classrooms 1 and 2 only.

Children were observed eating another children's lunches and a separate time a child was observed playing with outlet plugs, taking them in and out; both times without staff noticing.
