Findings: Staff 4 left their classroom unsupervised in order to open the door for the Surveyors to enter the facility. Surveyor 2 observed Staff 4 leave their classroom unsupervised 8 times in order to retrieve children entering the facility and to retrieve lunches from the lunchroom. Surveyor 2 also observed Staff 4 answer the telephone while supervising two children. There was no staff available to provide the Surveyors with documentation that is required for review for a compliance inspection.

Findings: Surveyor 2 observed Staff 4 leave her classroom 8 times leaving two children in the school-age classroom without adult supervision. Surveyor 1 observed a preschool child go to the bathroom by themselves without supervision en route to and from the bathroom.

Worst place I ever worked, Jessica pays shit, she is mentally unstable and sad to say she de escalate situations
She still owes me back pay and have never gotten my money
She is always cutting corners and covering things up
She should not be near kids
