|Findings: Upon review of the personnel file of Staff 2, start date 10/14/2019, Surveyor 2 noted that the file did not have documentation to verify the submission of a request for a Central Registry background check.|
|Findings: Upon arrival to the facility, Staff 1 came in from the outdoor activity area to open the front door for the Surveyors. Staff 1 left 4 children in the outdoor activity area with Staff 3 and Staff 4. Neither Staff 3 nor Staff 4 were qualified to supervise children.|
|Findings: Upon review of the facility's parent handbook, Surveyor 1 noted that the following information was not included: -the hours of operation; -the facility address, city, state, zip code and phone number; -transportation procedures; -field trip requirements; -a statement that the licensee carries liability insurance and that documentation of the coverage is available for review on the premises; -a statement that inspection reports are available on site; -a statement that the facility is regulated by the Arizona Department of Health Services including the Department's address and phone number; -the procedures for notifying parents at least 48 hours in advance before a pesticide is applied.|
|Findings: Upon review of the facility's sign in/out records, Surveyor 2 noted the following: -Child 1 was not signed out on 5/23/2020 nor on 5/24/2020; -Child 2 was not signed out on 5/23/2020; -Child 3 was not signed out on 5/21/2020.|
|Findings: Upon review of personnel files, Staff 1 reported that two volunteers present at the facility on the day of the inspection do not have staff files at the facility.|
|Findings: When Surveyors arrived at the facility, there was not a staff member present in the front office to open the front door. Staff 1 left the outdoor activity area to come into the facility and open the door for the Surveyors, leaving 4 enrolled children with 2 unqualified staff members in the outdoor activity area. While Surveyors were conducting the inspection, the infant room teacher required assistance with bottle preparation for one of the enrolled infants. The teacher opened the infant door to call for another staff member, however, all staff were in the outdoor activity area and unable to hear the infant teacher.|
|Findings: Upon inspection of licensed activity areas, Surveyor 2 observed a can of Lysol on a counter that was accessible to enrolled children.|
|Findings: Upon arrival to the facility, Surveyor 2 noted that the entrance door to the infant room was open. There was 1 teacher and 2 infants present in the activity area.|
|Findings: Upon inspection of the infant activity area, Staff 2 reported to Surveyor 1 that infants remain in their cribs for most of the day. Surveyor 1 explained that the rule states that an infant should not remain in their crib if they have been awake for more than 30 consecutive minutes.|
|Findings: Upon inspection of the infant activity area, Surveyor 1 observed Child 4 on their stomach in a crib. Staff 2 reported that they placed the infant into their crib, on their stomach because that is what the infant is used to. Staff 2 reported that Child 4 is unable to turn over on their own. The facility did not have written instructions from the infant 's health care provider stating that they could be placed into a crib on their stomach.|
|Findings: Upon review of the posted infant feeding instructions for enrolled infants at the facility, Surveyor 1 noted that the instructions for Child 1 did not list any foods that could be given to the child. Staff 2 reported that they feed the infant various types of baby food from jars provided by the infant 's parents.|
|Findings: Upon inspection of the infant activity area, Surveyor 1 noted that there was a bottle that Staff 2 reported belonged to Child 1, that was not labeled with Child 1 's name.|
|Findings: Upon inspection of the Toddler activity area, Surveyor 1 noted that the container used for soiled clothing did not have a liner.|
|Findings: Upon inspection of the infant activity area, Surveyor 1 noted that written diaper changing procedures were not posted in the room. Staff 2 reported that they had fallen off the wall.|
|Findings: Upon review of the facility 's snack menu, Surveyor 2 noted that on 5/6/2020 the facility 's snack menu listed water and Cheez-It crackers and on 5/8/2020 it listed water and pretzels. The facility served one meal component, however, two components are required for snacks.|
