Description: 12 months prior to service and every 24 months thereafter

Noncompliance Area: At the time of the inspection, the health assessment on file for staff person #1 was not updated every 24 months, as required. This is evidenced by the current health assessment date of 3/24/26 and the previous health assessment date of 10/2/23.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Staff person #1 has not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): PQAS approved Pediatric First Aid/CPR training.

Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)10. Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a child-care position at the facility.

Description: Supervised at all times /Staff assigned to specific children

Noncompliance Area: On 8/13/25, Child #1 removed a cup of Ramen noodles from the microwave, which spilled on the child's stomach, scalding the child and causing immediate blistering. At this time, Staff #1 and #2 were assigned to this classroom and a group of 21 school-age children. Staff #1 acknowledged that Staff #2 had left the classroom to put towels in the dryer, leaving Staff #1 alone with 21 children. Staff #1 was dealing with an incident with another child at the time the child was scalded.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Tiered LIS: 1.) Children must be supervised at all times. This portion of the plan requires an immediate correction date. 2.) The Legal Entity / Director will establish a supervision and ratio policy. The policy will include how the facility will demonstrate their knowledge and ability to adequately supervise children while maintaining appropriate ratio at all times. The policy must focus on ensuring that each child is seen, heard, assessed and able to be directed at all times. The policy must also include a plan for each staff to be assigned the responsibility for supervision of specific children, including the requirement that the staff person know the names and whereabouts of the children in his assigned group and be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The policy must include procedures to maintain proper supervision and ratios when a staff person needs to leave the classroom for any reason (i.e. to complete an errand, to use the restroom, or for any other reason). This supervision and ratio policy must be submitted to the Department prior to implementation for approval. Once approved by the Department, the director will ensure that all current employees and any new hires are trained on this policy, and proof of that review will be kept on file at the facility. This date of correction will be when all requirements have been met.

Description: Similar Age Level

Noncompliance Area: Staff Person #1 acknowledged that they were alone in the classroom with 21 school-aged children when an incident occurred on 8/13/25 to Child #1. This number and age of children requires there to be two staff persons present at all times.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. TIERED LIS: 1.) The required staff:child ratios must be maintained at all times. This portion of the plan requires there to be an immediate correction date. 2.) The legal entity must print and post the required ratios in each measured child care space. This portion of the plan will be when the required ratios are posted in each childcare space.

Noncompliance Area: On 8/13/25, Child #1 removed a cup of Ramen noodles from the microwave, which spilled on the child's stomach, scalding the child and causing immediate blistering.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F. The policy must also address microwaves, and their usage in the classroom, by staff and children. The legal entity will submit policy to the Northeast Regional Office for approval. Once approved, all existing staff and new hires must be trained on the approved policies and procedures, and it must be documented at the facility. This date of correction will be when all requirements have been met.

Description: Clean and good repair

Noncompliance Area: At the time of the inspection, the outdoor climber on the preschool playground had a plastic slide was cracked in numerous places by the bolts that secured it to the climber. This created rough edges and was a hazard to children.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Infant/toddler - no material less than 1 inch

Noncompliance Area: At the time of the inspection, pieces of wooden mulch, which came from the preschool playground and measured less than 1 inch in diameter, were observed on the toddler playground.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care.

Description: Potentially hazardous food refrigerated

Noncompliance Area: At the time of the inspection, a cheese sandwich was observed in a lunch box that was hanging on a hook in the school age room. This is required to be refrigerated.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Regulation: 3270.241(a)

Description: Requirements specific to school-age programs

Noncompliance Area: At the time of the inspection at 9:15am, child #1, a school aged child was observed sleeping in a corner of the room in the infant room. Child #1 was required to be in school at this time. Staff person #3, child #1's mother, acknowledged that her child should have been in school, but she was missing it due to her not sleeping the night before.

Correction Required: If a child is required to be enrolled in public or private school under the Public School Code of 1949 (24 P.S. §§ 1-101-27-2702) and if the child is not enrolled and if the child is not exempted from enrollment under the Public School Code, a child care facility may not admit the child for care during the hours when the child is required by law to attend public or private school.

Noncompliance Area: Staff person #4 has completed the Health and Safety Basics: Requirements for Certification (6 hours) training requirement within 90 days of hire; however, it was dated 8/24/18. This training date is outside of the approved dates of 9/30/2016 - 6/30/2018 for taking this training.

Correction Required: Staff persons shall complete professional development titled "PA Required Health and Safety for Centers-Group Child Care Part 1 and PA Required Health and Safety for Centers-Group Child Care Part 2." Until such time as the required training has been completed, staff person #4 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #4, staff person #4 may not work in a child-care position at the facility.

Description: Hazardous toys and equipment

Noncompliance Area: A Fisher Price Infant to Toddler Rocker was observed in the Infant room. The United States Consumer Product Safety Commission deemed this hazardous in June 2022 and may not be used by children at the facility and may not be on the premises at the facility.

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement.

Noncompliance Area: The health assessment on file for child #1, a preschool child, did not include a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Description: Surfaces cleaned

Noncompliance Area: During the inspection on 8/23/24, in the Toddler room, due to the changing table pad being taped where it was previously torn, it is unable to be properly cleaned by wiping the surface with a sanitizing solution.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.
