Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: The staff file for staff person #1 contained an updated health assessment dated 12/9/19 which was conducted more than 24 months from the previously dated 12/7/17 health assessment.

Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The child file for child #2, who is a young toddler, lacked an updated health report. The last health report on file is dated 4/13/16.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: The child file for child #1 contained a health report that lacked a statement that the child is able to participate in child care and is free from communicable disease.

Noncompliance Area: In the upstairs bathroom used by the children the following toxics were observed to be accessible to the children: a spray bottle of Scrub Soft cleaner on the window sill, a bottle of mouth wash and tube of toothpaste on the sink. In the living room used by the children the following toxics were observed to be accessible to the children: a pump bottle of hand sanitizer and a spray can of Lysol which were on the top shelf of the computer desk. A bottle of windshield wiper fluid was observed on the front porch which the children have access to as they enter/exit the facility.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: An ashtray full of cigarette butts was observed outside on the porch ledge nearest the front door which is used by the children to enter/exit the facility and is considered accessible to the children.

Correction Required: Ashes and cigarette or cigar butts are prohibited in a child care space, a play space or a food preparation area.

Noncompliance Area: The first-aid kit was observed to lack tweezers.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
