Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In the kitchen, the refrigerator had 7 bottles without a date, 1 sippy cup and 1 additional bottle without a name or date.

All equipment and furnishings were not in good repair. Several outside play structures are rusted. All rust will need to be removed and or painted over.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Documentation was not on file for one staff member.
