Corrective Action: Sleep checks must be conducted and documented every 30 minutes. Staff members are to observe the infant for normal breathing, signs of distress, and to be ready to respond in case of emergency. At no time during a three-hour period did either staff member conduct a 30-minute safe sleep check. The licensee will ensure all staff members complete the DIEEC training Infant Safe Sleep Practices for Childcare Providers.

Corrective Action: Disposable covers must be used for each diaper change. Licensing specialists observed that neither staff member used disposable covers for any of the diaper changes conducted on January 20, 2020. Hand washing for the person who changed the diaper and for the child with soap and water is required immediately after each diaper change. Licensing specialists observed that neither staff member washed their hands or any of the children’s hands for any of the diaper changes conducted on January 20, 2020. Cleaning with soap and water and then disinfecting the diaper-changing area after each use is required. Licensing specialists observed that neither staff member cleaned or disinfected the diaper-changing area for any of the diaper changes that were conducted on January 20, 2020. The licensee will ensure Delacare Regulation #61 is reviewed with all staff members and clean and disinfect the infant classroom.

Corrective Action: Infants are to be fed on demand or according to the infant feeding schedule provided by the parent/guardian. Licensing specialists observed the staff members feeding all of the infants at the same time. An infant must be held for bottle-feeding. When an infant is able to hold a bottle or does not want to be held while fed, the infant may be placed in a high chair or at a feeding table. Licensing specialists observed the staff members feeding the infant while both staff and infants were sitting on the floor. An infant must never be placed in sleeping or relaxing equipment with a bottle or have a bottle propped for feeding. Staff #2 was observed attempting to give a child a bottle while the child was laying in the crib. The child refused the bottle and the bottle was removed from the crib. The licensee shall ensure Delacare Regulation #72 is reviewed with all staff members working in the infant classroom.

Corrective Action: All staff members are required to be trained and follow safe sleep practices in the infants’ classroom. Infants must be placed in a crib, with a firm mattress, with only a tight fitting sheet. Staff #2 was observed placing a blanket in the crib and tuck it around the crib mattress before placing the child in the crib for nap. Infants must be placed into the crib on their backs. Staff #1 and Staff #2 were observed placing two children into the crib on their stomachs. The center will ensure all staff members complete the DIEEC training Infant Safe Sleep Practices for Childcare Providers.

Corrective Action: A licensee shall ensure each staff member has an understanding of and respect for children and their needs. Video footage of the infant classroom was reviewed during the complaint investigation. Licensing specialists observed Staff #1 handling Child #1 in a careless manner which resulted in the child being injured. The licensee will ensure that all staff receive training on how to properly handle and pick up a child.
