I have used this provider for more than 6 months

Children has continually been diagnosed with Hand, Foot, and Mouth Disease. The daycare hasn't been up to par with letting parents know about this contagious disease and let's the children remain in the preschool while knowing of the condition while spreading to other children. A known child of a family member of mine has just recently tested positive for this disease and the daycare hadn't sent out any information that a child was diagnosed just a week prior told from another parent. Have witnessed children being pushed to the ground by older daycare workers. Not good how can you maintain a four-star rating under the North Carolina Department of Health and Human Services? While the state is paying anywhere from $440 to $600 per child for some parents.
This is not good at all and will be looked into

In the adult bathroom located in the hallway that was unlocked there was a Febreze aerosol air freshener spray can below 5 ft, under the bathroom sink that was not locked.

All equipment and furnishings were not in good repair. Shelving and nap time dividers had worn contact paper, duct tape, and glue residue.

A safe indoor and outdoor environment was not provided for the children. The children were observed playing in an unfenced grassy area that had landscape timbers stacked on on top of each other. In a fenced area used by toddlers I observed stairs that were accessible tot he children. broken glass and sharp stumps were accessible tot eh children. In the toddler classroom, staples were protruding from the wall.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. An emergency medication for asthma was stored in a bookbag less than five vertical feet from the floor. The teacher placed the medication on a shelf above five feet from the floor during myb visit.

Parent's medication authorization did not include required information. Permission to administer a medication did not have specific instructions regarding how to administer it, or how medicine to administer.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One medication which was dispensed 1/25/2023 to be administered for fourteen days, was still on the premises.

All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Six staff files did not have a current Health Questionnaire on file for review.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Six staff files did not have a current Emergency information form on file for review.
