Noncompliance Area: The file of facility person #1, hired 4/19/19, does not contain completed NSOR clearance and does not contain a copy of a request NSOR clearance a signed employee disclosure statement required for provisional hiring under the CPSL.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 1 may not work in a child care position at the facility.

Noncompliance Area: Observed 3 out of 4 sleeping mats in the infant room not to be marked for a specifics child's use.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: On 6/11/19 at approximately 9:15 AM staff person #1 did not know the names of the children specifically assigned to her for purposes of supervision. She had 8 children assigned to her care but could only name 7 of the children.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Children's files were reviewed, children #1 and #2 did not have written consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: Observed multiple bolts on the fence surrounding the outdoor play space to be protruding into the children play areas.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed the large piece of embedded equipment in the outdoor preschool play area not to be installed over a sufficient amount of protective covering

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Children's files were reviewed, child #1 did not include written consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: Staff #5 did not administer prescription medication as per written instructions on the medication label.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

Noncompliance Area: Children's files were reviewed, children #1, 2, 3. and 4 did not have updated financial agreements on file.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Staff files were reviewed, staff persons #1, 2, and 3 did not receive training on the emergency plan annually.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: Staff files were reviewed, staff persons #2, 3, and 4 did not receive annual fire-safety training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: Observed a Magic Eraser to be present in one of the cubbies located near the door in the Head Start room. The Magic Eraser was accessible to children receiving care in that room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Observed the toilet seat to be loose in the PS-C room.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed the small Rubbermaid shed, closest to the toddler playground , which stores the children's outdoor equipment, to be in disrepair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed the mulch underneath the large piece of outdoor equipment not to be loose fill. The mulch was compacted and no longer loose fill.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.
