Noncompliance Area: 1. During the physical site inspection, protruding bolts were observed on the fencing of the new outdoor natural play space. 2. During the physical site inspection, on the shed off the preschool exit to the outside play area, the bottom trim was observed to have worn causing sharp wood pieces. 3. During the physical site inspection in the preschool play area, the bottom of the fencing was observed to be coming apart with sharp metal pieces exposed.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: No peeling paint or plaster

Noncompliance Area: During the physical site inspection, peeling paint was observed on the shed off of the preschool exit to the outdoor play space.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Description: Disposable diapers

Noncompliance Area: The diaper can in the young toddler room was observed to not be functioning in a hands-free way, the lid not rising when the pedal was depressed.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Description: Health assessment, TB test

Noncompliance Area: Record for Staff #1 was observed to contain a current health assessment but was dated outside the required 24-month period. (4/25/2022, 12/20/2024). Corrected at time of inspection.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Description: Building Surfaces

Noncompliance Area: In the outdoor play space, the concrete near the water drain by the SACC space door was observed to have a significant growth of green slimy organic material accessible to children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: Lighting

Noncompliance Area: The all-purpose area used by the SACC program near the reception area was observed to have partially non-functioning overhead lighting causing the room to be quite dim.

Correction Required: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light.

Noncompliance Area: First aid kits were observed to be missing the following: PS-C (no scissors, no tweezers), Jungle Room (no tweezers), Explorers Room (no scissors)

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Description: Building Surfaces

Noncompliance Area: The SACC room refrigerator was observed to be in need of cleaning. The outside playground on the toddler side was observed to have disintegrating padding around the poles, and the playground was observed to have a heaved section of sidewalk creating a trip hazard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: No peeling paint or plaster

Noncompliance Area: The SACC room was observed to have several holes in the wall near the light switch, and peeling paint in the trim at child level. PS-A was observed to have flaky paint in the trim near the door.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Regulation: 3270.31(e)(4)(i)

Description: Age and Training

Noncompliance Area: During record review portion of the Renewal Inspection, Staff #1 did not update her Pediatric First-Aid and Cardiopulmonary resuscitation certification.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Noncompliance Area: The file of facility person #1, hired 4/19/19, does not contain completed NSOR clearance and does not contain a copy of a request NSOR clearance a signed employee disclosure statement required for provisional hiring under the CPSL.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 1 may not work in a child care position at the facility.

Noncompliance Area: Observed 3 out of 4 sleeping mats in the infant room not to be marked for a specifics child's use.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: On 6/11/19 at approximately 9:15 AM staff person #1 did not know the names of the children specifically assigned to her for purposes of supervision. She had 8 children assigned to her care but could only name 7 of the children.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Children's files were reviewed, children #1 and #2 did not have written consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: Observed multiple bolts on the fence surrounding the outdoor play space to be protruding into the children play areas.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed the large piece of embedded equipment in the outdoor preschool play area not to be installed over a sufficient amount of protective covering

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Children's files were reviewed, child #1 did not include written consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: Staff #5 did not administer prescription medication as per written instructions on the medication label.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

Noncompliance Area: Children's files were reviewed, children #1, 2, 3. and 4 did not have updated financial agreements on file.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Staff files were reviewed, staff persons #1, 2, and 3 did not receive training on the emergency plan annually.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: Staff files were reviewed, staff persons #2, 3, and 4 did not receive annual fire-safety training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: Observed a Magic Eraser to be present in one of the cubbies located near the door in the Head Start room. The Magic Eraser was accessible to children receiving care in that room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Observed the toilet seat to be loose in the PS-C room.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed the small Rubbermaid shed, closest to the toddler playground , which stores the children's outdoor equipment, to be in disrepair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed the mulch underneath the large piece of outdoor equipment not to be loose fill. The mulch was compacted and no longer loose fill.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Observed a blue couch located in the room utilized by head start to have the foam interior exposed.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child`s size, age and special needs.

Noncompliance Area: Staff files were reviewed, staff person#6 did not have verification of experience on file.

Correction Required: A facility person`s record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: Staff files were reviewed, staff persons #4, #5, and #6, did not contain 2 non family written references.

Correction Required: A facility person`s record shall include two written, nonfamily references from individuals attesting to the person`s suitability to serve as a facility person

Noncompliance Area: Staff files were reviewed, staff person #1 did not review the emergency plan annually.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility.

Noncompliance Area: Staff files were reviewed, staff persons #2 and #3 did not receive annual fire safety training.

Correction Required: Toilet areas and fixtures shall be cleaned daily and be in good repair.

Noncompliance Area: Observed an older toddler playing in the outdoor play space to have several small stones in a child-sized cart. Children receiving care in the outdoor play space had access to small stones in a adjacent area.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The operator did not insure staff members #1 through #4 did not receive training regarding the emergency plan at the time of their initial employment.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.
