Jumpstart Childcare and Learning Center is great!  They provide excellent, personable, professional service and I can't say enough about them.  Their staff are friendly, knowledgeable, experienced and their facilities are clean and very well maintained.  I wouldn't trust my childs care to anyone else.

The following was discussed but not limited to: Infant feeding plans Wood table on playground Infant room staff annual training The Notice of Inspection Rights were provided to the licensee at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection.

Initial Comments: **AMENDED** The following deficiencies were observed at the time of the Compliance Inspection conducted on 10/09/2024 and are subject to changes pending programmatic review. Submit the Plan of Corrections using the AZDHS Licensing Portal within ten days from the date the Statement of Deficiencies is received. The Empower Self-Evaluation survey was emailed to the licensee at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: DCS Central Registry check: Submit the application and print the results using the Arizona Centralized Background Checks website (cbc.az.gov). Director change: Update the Director using the AZDHS Licensing Portal. Criminal History Affidavits: Complete all Criminal History Affidavits to include the CDC or address. The Compliance Officer is Tara Farrell.
