Description: the staff worker did not count the children and make a face to name attendance therefor leaving an eight-year-old child in the restroom unsupervised for 5-8 minutes when she took the rest of the group to the third floor rooftop play space. The eight-year-old child went from the second floor to the first floor looking for her group. The same staff worker failed to take the attendance clipboard with her when she took a group of three-year old children on a walk outside during the week of August 10, 2020.
