Per the center director, Child #1's Epipen is likely located inside the school office however it is unable to be accessed if needed as the office is observed closed during the visit. Violation

2025-01-14 On 1/6/25 staff failed to notify Child #2's parent of scrapes the child received while engaged in a game of hide and seek with program staff after the child climbed inside a garbage can and sustained injury while attempting to exit the garbage can. The child is also reported to have cried for 5 minutes following the incident. The parent was not informed of the incident & injury until the following day when the parent called the director to inquire what had happened to her child and informed the director that she had taken the child to the emergency room for professional medical evaluation. 2025-01-14 An incident occurred on 1/6/25 in which Child #2 was injured in care while exiting a garbage can which resulted in Child #2 receiving professional medical evaluation and a determination that the child had swelling around one of her ribs, two quarter sized bruises, a "tissue bulge on her rib cage" and a two inch cut on her forearm with swelling and bruising. This was never recorded in the medical book. Violation
