Noncompliance Area: On 11/4/20 observed Staff numbers; 1-2-3 & 4 outside within the play area with 10 young toddlers and 12 older toddlers. Staff were not wearing face coverings. These staff were not able to maintain the 6 foot social distance between them and the toddlers and were not maintaining the 6 foot social distance between themselves.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the child. Effective 7/17/20 the CDC Guidance outlined four Critical Elements for day cares that include critical planning and activities which should occur in child care settings to protect the health and safety of both children and staff. The first outlined critical component denotes; "Child care staff must wear face coverings indoors; they must also wear face coverings outside where staff are unable to maintain the 6-foot social distance, unless a medical reason prevents the staff from wearing a face covering. This includes anyone who has trouble breathing, is unconscious, incapacitated or otherwise unable to remove the face covering without assistance.

Noncompliance Area: Staff number 1 start date as staff 3/22/19 does not have a valid FBI clearance on file at the facility

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). . LACKING REQUIRED HIRING DOCUMENTS: . A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #-1-- may not work in a child care position at the facility.

Noncompliance Area: 6/19/18 observed staff number 1 with a group of 12 pre-school children. Staff number 1 verified that she was assigned to this group of 12 pre-school children.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. The required staff:child ratios must be maintained at all times. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3270.51--3270.55.
