Initial Comments: The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on April 23, 2026 and are subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The following was discussed but not limited to: - License renewal - Licensing outdoor playground area - Janitorial item storage - Personal products - Fire extinguishers - Diaper changing containers - Fire drills - Accident, Emergency and Evacuation form
INSP-0163491 2025-12-01 Complaint Complete
Initial Comments: The purpose of the inspection was to conduct a Complaint Investigation for Case #00150673 from December 01, 2025 - December 18, 2025. A full inspection was not conducted at this time. The following classroom ratios were observed: *2:5 Infants *1:4 Ones *1:5 Twos/Older *1:5 Twos/Older *1:8 Three/Older *1:2 Fours/Older There was one fingerprint clearance card verified on the DPS website during the investigation. There were six staff members interviewed during this complaint investigation, one of which was via phone while onsite for the investigation. The Compliance Officer contacted the original Complainant to gather additional information and medical documents. Documentation observed included one staff file, one Emergency, Information and Immunization Record (EIIR) card, one written Incident Report dated October 02, 2025, child sign in/out documents, staff sign in/out documents, classroom attendance rosters, the facility's current Certificate of Liability insurance, one medical report, and various email correspondence regarding the incident. Upon completion of the complaint investigation it was determined from documentation and interview that one allegation on Case #00150673 was able to be substantiated at this time. The other allegation was unable to be substantiated at this time. The following deficiency was found during the complaint investigation, subject to changes pending programmatic review. Please complete and submit a Plan of Correction via the online portal within 10 days of receipt of this Statement of Deficiency. Items discussed, but not limited to, were as follows: -Liability insurance coverage and requirements -Updated process DCS Background Check process -Individualized Plan available on website -Document medical care if informed by parent/guardian -Document disenrollment date on EIIR card -Mandated reporting -New forms and rule set available on AZDHS website

Initial Comments: The following deficiencies were found at the time of the initial monitoring inspection conducted on July 24, 2025, and are subject to changes pending programmatic review. Note: A full inspection was not conducted. Eight of eight fingerprint clearance cards were verified to be valid on the DPS website during the time of the inspection. Initial Comments: The following deficiencies were found at the time of the Initial Licensing Inspection conducted on 05/19/2025, and are subject to changes pending programmatic review. #11 of #11 fingerprint clearance cards were verified to be valid via the DPS website during the time of the inspection. Please complete the Plan of Corrections via the online Portal within 10 days of receipt of this Statement of Deficiencies. Please submit a photo of the corrected deficiencies as requested. The facility was not licensed at the time of the inspection.
