The Complainant was unable to be interviewed. The Yuma Police Department Detective was interviewed as part of the investigation. Three staff/residents were interviewed as part of the investigation. Documentation reviewed included child enrollment forms, attendance records, and a parent medication authorization letter. Based on interview, documentation, and the Compliance Officer and Compliance Officer Supervisor's observations, the Department was unable to substantiate that the one allegation occurred on the home's premises. Please submit documentation of the following staff file items prior to re-opening from temporary closure: *Staff #1: Good-faith references, *Name, address, and telephone number, Criminal History Affidavit, immunization statement, TB skin test results, good faith references, and Central Registry background results. | |||
