Corrective Action: Provide signed parent permission to sleep in a separate area for all children enrolled. Provide documentation that 15 minute sleep checks are being completed when children are sleeping in a separate area. The provider must follow safe sleep practices. An infant was seen asleep in a pack-n-play, which was not on the lowest setting, with a blanket and a bottle. The blanket and bottle were removed and the provider was instructed to adjust the mattress to the lowest setting. This was corrected at the visit.

Filthy dirty facility your child will be sick they're also running a facility with RSV children present. Trisha is a thief and very rude
