The White Oak office is open from 7:00am until 4:30pm Monday through Friday. If you need to register a student, you should bring the following items with you: the child's birth certificate, the child's immunization record, and proof of residence (i.e. an electric or water bill showing your name or address). The child must accompany you if you are registering them for kindergarten. If you child has been in school somewhere else, you will also need to bring their last report card from that school with proof of grade placement. Also, if the child is not your biological child, you must bring proof of legal guardianship. Children entering kindergarten must have a current physical documented on a form from the school. Please call the school office at 221-4078 to set up an appointment to register a child.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space B107 an electrical power strip beside the refrigerator was missing three(3) outlet covers, the outlet located under the television was missing and outlet cover, and the outlet located near the cots was missing an outlet cover.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space B108 an aerosol can of Lysol disinfectant spray, 2 cans of Barbasol shaving cream, a box of 20 Mule borax, and 2 containers of Clorox wipes were stored in the unlocked cabinet above the sink. In space B107 a packet of Vita Globe Super Probiotics Dietary Supplements was stored on the side of a lunch bag that was sitting on top of the mini refrigerator.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space B107 a tube of Orajel was in a lunch bag on the shelf beside the Cozy Corner.
