Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #1 - may not work in a childcare position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Description: Restraints prohibited

Noncompliance Area: At time of inspection, twin infant children were inside car seats for over 30 mins while at facility which put them in a confined space and restricted their movement.

Correction Required: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

Noncompliance Area: At time of inspection, the first room had 6 children (4 PS and 2 infants) with one staff person which put her classroom out of child/staff ratios until the second staff came into the facility. When including the other classroom there was a total of 12 children in care putting the facility over the capacity by 2 children.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements at §3280.52(b).
