One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at §3290.73(c).

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Noncompliance Area: During renewal inspection on Tuesday, 04/26/22, cert rep observed provider failed to have a written plan identifying the means of transporting a child to emergency care and staffing provisions, posted in area used by parents.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: During renewal inspection on Monday, 05/06/19, cert rep observed facility's emergency transportation plan failed to include the facility staffing provisions in the event of an emergency.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: In review of children's files, child 2, young toddler, failed to have and updated health report contained in file. Health report contained in file was completed on 01/26/18.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: In review of facility person's files, facility person 1 failed to have a health assessment contained in file.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment on file at the facility. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of facility person's files, facility person 1 failed to have results of tuberculosis screening by the Mantoux method contained in file.

Correction Required: An adult health assessment shall include Tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, CRNP, the Department of Health or a local health department. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of facility person's files, facility person 1 failed to have signed disclosure statement and mandated reporter training contained in file.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: During renewal inspection on Monday, 05/06/19, cert rep observed facility's emergency plan failed to provide a shelter away location during an emergency at the facility.

Correction Required: The facility shall have an emergency plan that provides for shelter of children during an emergency including shelter in place at the facility and shelter at locations away from the facility premises..

Noncompliance Area: During renewal inspection on Monday, 05/06/19, cert rep observed facility's emergency plan failed to provide a method of contacting parents at the beginning and end of emergency.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Noncompliance Area: Facility persons had not been trained on the facility's emergency plan at the time of the renewal inspection on Monday, 05/06/19.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training. A facility person's record shall include records of training required by the Department.

Noncompliance Area: In review of facility person's files, facility person 1 failed to have verification of age contained in file.

Correction Required: A volunteer shall be 16 years of age or older and shall be directly supervised at all times.

Noncompliance Area: The pack-n-play used by the facility children to sleep was no labeled for individual use at the time of inspection.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: There was no or no current fee agreement in files for facility children #2 and #3 at the time of inspection on 5/21/18.

Correction Required: An agreement shall specify the amount of the fee to be charged per day or per week.

Noncompliance Area: The emergency contact information in the file for facility children #1 and #2 were no updated in the past six-months.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information

Noncompliance Area: There was no hands-free trash can in the diapering area at the time of inspection.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: Peeling paint was observed along the wall and base board in the kitchen near the living room entrance.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
