|Findings: Complaint: The teachers were in the process of entering the building before they realized there was a child left hiding in the playhouse. The teachers were not aware of the child that was hiding in the playhouse until she called out to her mother, who had witnessed the situation take place. All teachers stated that a head count is usually done before they enter the building but on this particular day, the procedure was not followed. Teacher B stated as a result of this situation that took place, they will not make sure they enter together and always do a head count before entering the building. This specialist informed the teachers it might be best to just take the children that need to go to the bathroom, in to use the bathroom then come back out so everyone can enter the building all together. This specialist also informed the teacher and the director, teacher should have their attendance records with them at all times to ensure a name to face check is being done. The director stated she told the staff to ensure they do a head count at each and every transition. See Remarks.|

This specialist spoke to all 3 teachers that were present on this day. The teachers stated one child needed to use the bathroom resulting in Teacher A leaving the group with one child. At that point, the remaining teachers decided to all go back inside the building as well. The teachers state Teacher B was leading the group of children inside while Teacher C was rounding up a couple of children that were still attempting to play. At that point, Teacher B was in front of the line when another child mentioned she had to use the bathroom, so she proceeded to enter the building instead of waiting to enter all together. When Teacher C at the back of the line was walking away from the playground area, she stated that she heard a child call out to her mother. The teacher states it was at that moment she realized there was a child in the playhouse hiding. See Remarks.|
