| Initial Comments: The following deficiencies were found at the time of the initial inspection conducted on July 07, 2025, and are subject to changes pending programmatic review. There were seven personnel files reviewed. All seven fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Complete and submit a Plan of Corrections via the online Portal within 10 days of receipt of this Statement of Deficiencies. ** Submit pictures with your corrections for the following items: * Gaps under the fence that have been fixed and show no more than a four inch gap. * The missing fence area around the school-age playground area. * Playground climbing equipment age limitation information. * The shelving units secured to the walls. Items discussed, but not limited to: * Staff files: new start dates, 10 day trainings, and Criminal History Affidavits. * Stock medications in first-aid kits. * Field Trips * Rosters Licensure is pending upon receipt and approval of requested documents/pictures and the Plan of Corrections.
