Noncompliance Area: At the time of the inspection, there was not documentation of the provider reviewing the emergency plan annually.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. Each review and update of the emergency plan shall be documented in writing and kept on file at the facility.

Description: Comply with CPSL/CPSL information

Noncompliance Area: The file of staff #1 lacked current PSP, Child Abuse, and NSOR clearances due to the fact the ones in the file were dated PSP 10-21-20 and Child Abuse 10-20-20. There was not an NSOR report in the file. The file of staff #2 lacked a current NSOR clearance due to the fact the one on file was dated 10-20-20.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Based on the smoke detection testing log, the detection system was tested more than 30 days apart due to the fact the dates were 6-9-25 and 7-10-25, which is 31 days.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Noncompliance Area: At the time of the renewal inspection, there was not a means to supervise the children through the use of an electronic monitor, camera, mirror, or other device or method so the operator can see, hear, direct and assess the children in real time at all times.

Correction Required: When there is only one operator present in the family child care home and the operator is unable to be physically present with the children because the operator is preparing snacks or meals for children in care or the operator is using the restroom, the operator may comply with the supervision requirement in subsection (a) through the use of an electronic monitor, camera, mirror, or other device or method so the operator can see, hear, direct and assess the children in real time at all times.

Noncompliance Area: The file of child #1 lacked a child service report every 6 months due to the fact the most recent one on file was dated 4/2024.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).
