The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The Empower Self Assessment was emailed to the provider. The fingerprint clearance cards for 6 of 6 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring there is 6 inches of resilient surfacing for all fall zones on the playground. 2). Ensuring that toilet bolts are cut or covered. 3).
