2026-03-09 Actions were potentially injurious when staff pulled on a toddler's arm and another toddler's leg to get them into a laying position in their cribs and pack n plays. Additionally, staff was observed putting a sleep sac on a child roughly and later, approached the same toddler and pulled the sleep sac between their legs, in order to get the child in a laying position on their cot. Additionally, staff was observed picking up a toddler and roughly putting them into a pack n play to sleep. Violation

2026-03-09 On March 3, 2026, staff was forcing 5 toddlers to take naps when the staff person was pulling on one toddler's arm, another toddler's leg, and pulled on another toddler's sleep sac to get them into laying positions on their sleep surfaces. Additionally, a staff person was heard yelling at toddlers to "lay down". Violation
