Children were not adequately supervised at all times. A two year old child was in the bathroom washing her hands unattended as one teacher was assisting other students put shoes on to go outside - other teacher was already outside with a group of children. Child was in the bathroom area approximately 2-3 minutes alone. The caregiver, who was already outside, was observed looking at cell phone several times for several minutes throughout the observation as two year children played.

Electrical outlets, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Uncovered outlets were observed in space #2.
