The sponsor or sponsor representative shall designate individuals with the authority and responsibility to develop and implement written policies and procedures for the operation of the center, including a table of organization that illustrates lines of authority, responsibility and communication.

Violation Observed: Update the organizational chart with the names of current staff as well as the required job descriptions. | 7/24/19-The organization will decide who the Director and program supervisors are. Once these individuals are identified the program will do their table of organization.

The staff records specified in 3A:52-4.1(a) through (c) shall be maintained on file as follows: the Staff Records Checklist for the current director and all staff members currently working at the center shall be maintained on file at the center.
2019-06-06 3A:52-4.5(c)(1) Staff responsibilities
The director shall be scheduled to work at the center at least 50 percent of the center’s daily operating hours.

Violation Observed: At the time of inspection, staff reported that there was a change of Director recently. The new Director is only in attendance once per week at the site. As per OOL policy, the Director must be at the site 50% of the daily operating hours.
2019-06-06 3A:52-3.6(b)(1) Information to Parents document
The center shall distribute a copy of the Information to Parents document to each child’s parent(s) upon enrollment and to every person upon becoming a staff member and secure and maintain on file a record of each parent’s and staff member’s signature attesting to receipt of the document. The center shall maintain on file a copy of the Information to Parents document.
