Description: The file for Child B lacked a complete health history and emergency care plan upon licensing review. File noted a medical condition, the form lacked information pertaining to the medical condition and procedures to follow in the event of an emergency.

Description: The implemented tracking procedure failed at the time of licensing visit. Staff present in the preschool room was unaware of the names and number of children in care at the time of licensing visit, staff stated seven children were in the classroom while only five were observed, staff was unsure of who the children initially were. The attendance sheet also inaccurately documented the children in care. A Warning Letter was issued November 25, 2019.

Description: Staff failed to use universal precautions when cleaning and protecting a child's superficial wound. A child received a scratch on their arm, which broke the skin, staff applied a damp paper towel to the child's arm. Staff was not wearing gloves nor did they clean the area with soap and water.

Description: A fire extinguisher, located in the school-age classroom with children present, lacked documentation of an inspection within the past year. Each extinguisher on the premise shall contain a label indicating its present condition and the date of the last inspection by a qualified person, inspected once a year. The label/tag present on the extinguisher was from 2019.

Description: Center failed to ensure any action, by commission or omission, or any occurrence related to the operation or maintenance of the child care center did not adversely affect the health, safety and welfare of a child under the care of the licensee. Child care staff working at the facility were observed working with children, in an indoor setting, without wearing proper protective facial coverings (masks), as required by the health emergency public order. Staff were observed with their masks under their chins with their mouths and noses exposed, or not wearing any form of mask.

Description: The above floor level diapering surface did not contain a barrier or restraint to prevent falling. A mat was placed upon a heating unit to change children.

Description: Soiled diapers were not placed in a plastic-lined, hands-free, covered container at the time of licensing visit. Diapers were observed being disposed of in an open waste basket, unit was not covered during licensing visit.
2021-02-18 251.06(2)(a) Potential Source Of Harm On Premises
Description: The premise was not free from potential sources of harm at the time of licensing visit. An unlocked closet in the preschool room contained multiple hanging cords, posing a strangulation hazard to children. A heating unit grate was disconnected, allowing access to sharp edges. The basement room in which infants were being cared for contained loose cords and a heating unit without a side panel, exposing the inner mechanisms. Hanging cords were accessible to children in the 2-3-year-old room, from the video game system and television. A Warning Letter was issued November 25, 2019.
2021-02-18 251.06(2)(gm) Premises - Well Drained, Clean
Description: The premise was not clean and in good repair at the time of licensing visit. A hole was observed in the preschool closet door, the cabinet base panel in the basement infant area was disconnected, the heating unit in the basement infant room was missing the side panel to cover the interior heating unit.
