| Initial Comments: The purpose of the inspection was to conduct a Complaint Investigation for case #00177028 on July 28, 2026. Note: A focused inspection was conducted. The Primary Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the beginning of the inspection. Ratios observed were: Infant A: 2:7 Infant B: 2:10 Toddler A (ones): 1:6 Toddler B (ones & twos): 2:12 Twaddlers: (twos) 2:13 Preppers (twos & threes): 2:13 Preschool 1 (threes): 2:19 Preschool 2 (threes, fours and fives) 2:19 There were ten staff members interviewed during this investigation. Documentation observed was: Classroom and attendance rosters, staff and children sign in and out. Upon completion of the Complaint Investigation, it was determined from observation, staff interviews, and documentation that the allegation was unable to be substantiated due to a lack of sufficient evidence.
