2026-05-06 In Twaddler A on 4/22 children opened and closed the door repeatedly. Staff B yelled at them but did not go to the door until a child was locked out of the room by another child. The girl cried and tried to get back inside. At least 10 seconds passed before Staff B went to the door to allow the child back in. In Twaddler A on 5/6 children repeatedly climbed on tables and a bookshelf, and a child was playing with a dirty garbage can lid for at least 2 minutes. Staff did not intervene. Staff in Prepper A on 4/22 did not see a child open the room door and leave. Violation

Parent sign in sheet showed Child 1 arrived to the center at 8:06 a.m., but no sign in sheet showed Child 1 signed in to any room prior to being signed in to Toddler C at 8:40 a.m. Violation

2026-05-06 Classroom attendance showed and licensors saw groups that exceeded minimum required staff-to-child ratios on several occasions. For example, review of attendance for April 29 and April 30 showed the Prepper A room was over ratio for over 15 minutes in the morning when Staff C worked alone with more children than legally allowed. On May 6 licensors saw 19 children who were younger than 3-years-old but at least 1-year-old in the Twaddler A classroom with only two staff. Violation

On April 22 in an unopened room, "Infant C", there were moldy crib mattresses in the bathroom and two crib frames also had mold. Violation
