| Findings: Supervision Concern, Serious Injury. The provider self-reported that an infant had fallen out of a high chair onto their face. The health department performed an investigation and determined a staff member had unbuckled an infant who was in a high chair and then tended to another child in the classroom. The infant fell out of the highchair onto their face and shortly after began vomiting. Resolution: The parents were notified and due to the severity of the incident, parents picked up the infant and took them to their pediatrician for an evaluation. The staff member in question was moved to another room and will complete infant safety courses. | |||
