Regulation: 3270.52 Description: Mixed Age Level Noncompliance Area: Per staff statements, on 9/16/21, staff #1 & 2 were supervising 13 children, 12 preschool aged children and one young school age child, in the outdoor fenced play space. Between the two staff they had only identified 12 children in their combined supervisory groups. At approximately 12:30 pm, when the groups left the play space, child #1 remained on the play space unsupervised for approximately 3 minutes while the staff and other children returned to the child care building. Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). |

Copy - child's file Noncompliance Area: On 8/27/21, Child #1 was bitten by another child in the classroom. The staff verbally notified Child #1's parent of the incident but failed to create a written report for the parent, facility and child's file. Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file. |
