All infant room staff have been re-trained on the correct sleep positions according to the Academy of Pediatrics. All staff know that all infants are to be placed on their backs to sleep. If an infant can roll on their own after being placed on their back, they may remain in that position. I will continue to monitor the sleep patterns of the infants and do frequent checks of the infant room staff to make sure they are following the proper procedures for putting sleeping infants down on their backs at all times.

Child #4 provided documentation of the 2nd dose of Hepatitis A immunization from the child's doctor. In the future, I will make sure all children have a paper stating they have had a flu shot for the year or that they object to the flu shot for their child if they object to getting it. The signed form will then be placed in the child's file. I will pay closer attention to make sure all children are current on all immunizations at all times.

In the future I will make sure that all medications brought into the center has a signed permission form from the parents stating that we are allowed to administer medication to their child.

Staff #1 obtained a new college transcript with the needed information on it. She also has a staff verification form filled out from a parent from Little Learning Land, Inc. verifying ownership of the center. Please see attachments for verification. I will make sure that all staff has proper documentation in the future!
