Findings: Upon arrival to the facility Surveyor 1 heard an infant crying loudly in the infant room. As the Surveyor entered the infant room they observed an infant in a bouncer crying. One staff was tending to a child in the rocker and the other staff was at the counter. When Surveyor 1 asked the second staff what they were doing they said they were entering a diaper change in the tablet. Surveyor 1 explained to the staff member that when an infant is crying out as the infant in the bouncer was, they needed to tend to the infant first.

Findings: Surveyor 1 observed Child 4 sleeping in a swing. The facility did not have written authorization from the child's physician. The infant was removed from the swing when the Surveyor asked about the sleeping infant.
