Description: SUCCESS ACADEMY FOR 3'S AND 4'S is a Five Star Center License in CAMDEN NC, with a maximum capacity of 96 children. This child care center helps with children in the age range of 2 through 12. The provider does not participate in a subsidized child care program.

A statement of receipt of the Shaken Baby/Abusive Head Trauma Policy was not included in the center's revised application and was not available for in two of six children's files reviewed.|
|All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. The storage closet in the hallway was unlocked. The closet was used to store cleaning products: Lysol Disinfectant Spray, Febreze, generic disinfectant spray, Zep Wipes, carpet cleaning solution, Goo-Off, and window cleaner.|

|The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Documentation of completion of Recognizing Child Maltreatment Training was not available for a staff person hired on 10/27/2021 and a staff person hired on 3/1/2022.|
|When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. A staff person administered an inhaler to a child but did not document it on a medication log.|
