Noncompliance Area: Observed children randomly being placed in highchairs during non-mealtimes with little or no activities provided.

Correction Required: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.

Description: Supervised at all times

Noncompliance Area: At approximately 1:25 PM and 2:15 PM Certification Staff observed Staff #1 leave their assigned group alone in the Buzzy Beehive Room. At 1:25 PM Staff #1 was observed in the hall by the office and at 2:15 PM Staff #1 was observed taking a child to the lobby to meet the child's parent.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Description: Staff assigned to specific children

Noncompliance Area: At approximately 1:25 PM and 2:15 PM Certification Staff observed Staff #1 leave their assigned group alone in the Buzzy Beehive Room. At 1:25 PM Staff #1 was observed in the hall by the office and at 2:15 PM Staff #1 was observed taking a child to the lobby to meet the child's parent.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Staff #1 did not ensure that a child's hands were washed after being diapered in the Buzzy Bee Hive Room. Staff #2 did not ensure that a child's hands were washed after being diapered in the Kiddie Kingdom Room.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: The files of Staff #2, #3 and #4 lacked a health assessment conducted within 12 months prior to providing initial service in a child care setting (SEE LIS Code Sheet). The file of Staff #2 did not contain a health assessment. The file of Staff #3 contained a health assessment dated 3/24/26. The file of Staff #4 did not contain a health assessment.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Description: Adult Hygiene

Noncompliance Area: Staff #1 did not wash their hands after diapering a child in the Buzzy Bee Hive Room.

Correction Required: A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Regulation: 3270.175(a)

Description: Age appropriate restraints

Noncompliance Area: Based on interviews, staff indicated children including infants, toddlers, and preschoolers were transported on a bus for a field trip without using car seats or booster seats.

Correction Required: A child 7 years of age or younger shall be transported in accordance with the requirements for parents and guardians as stated in 75 Pa.C.S. §4581 (relating to restraint systems).

Description: Two written references

Noncompliance Area: The files of Staff #2 and #4 lacked two written, nonfamily references from individuals attesting to the persons' suitability to serve as a facility person. The file of Staff #2 contained one written reference. The file of Staff #4 did not contain any references.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Regulation: 3270.21

Description: General Health and Safety

Noncompliance Area: Observed a cord to a baby swing which was not secured creating a potential tripping and/or strangulation hazard in the Kiddie Kingdom Room.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Regulation: 3270.25(a)

Description: Availability of certificate of compliance and applicable regulations

Noncompliance Area: The facility's current certificate of compliance was not posted in a conspicuous location used by parents.

Correction Required: The facility's current certificate of compliance shall be posted in a conspicuous location used by parents. The operator shall provide the parent of each child enrolled with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office.

Regulation: 3270.27(c)

Description: Training regarding plan

Noncompliance Area: The file of Staff #1 lacked documentation of receiving training regarding the emergency plan at the time of initial employment (SEE LIS Code Sheet). The file of Staff #1 contained no documentation of emergency plan training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Description: Comply with CPSL/CPSL information

Noncompliance Area: The file of Staff #1 lacked a National Sex Offender Registry Certificate prior to providing initial service in child care (SEE LIS Code Sheet). The file of Staff #1 contained a NSOR dated 12/16/25. The file of Staff #4 lacked a valid Child Abuse Clearance. Staff #4 had a volunteer Child Abuse Clearance dated 10/16/24.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #4 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #4 may not work in a child care position at the facility.

Description: Each staff person meets quals

Noncompliance Area: The file of Staff #4, who was designated as an aide, lacked documentation of meeting one of the applicable staff qualifications for the position in which the person was performing.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing.
